ABSTRACT

To explore the influencing factors of acupuncture curative effect in literature of experts' experience. The journal literature of experts' experience was retrieved from China National Knowledge Infrastructure (CNKI) and PubMed, starting from inception to September 4, 2020, and the influencing factors of acupuncture curative effect were extracted and analyzed. A total of 499 articles were included, involving 495 articles in Chinese and 4 articles in English. The influencing factors of acupuncture curative effect mainly include five aspects: diagnostic method, acupoint selection of acupuncture, acupuncture manipulation, regulating mind of acupuncture and acupuncture time, and provide reference for acupuncture protocol design in clinical trials.

Abstract Objective To evaluate the agreement between the histopathological diagnoses of preoperative endometrial samples and surgical specimens and correlate the agreement between the diagnoses with the impact on surgical management and the survival of patients with endometrial adenocarcinomas. Methods Sixty-two patients treated for endometrial cancer at a university hospital from 2002 to 2011 were retrospectively evaluated. The histopathological findings of preoperative endometrial samples and of surgical specimens were analyzed. The patients were subjected to hysterectomy as well as adjuvant treatment, if necessary, and clinical follow-up, according to the institutional protocol. Lesions were classified as endometrioid tumor (type 1) grades 1, 2, or 3 or non-endometrioid carcinoma (type 2). Results The agreement between the histopathological diagnoses based on preoperative endometrial samples and surgical specimens was fair (Kappa: 0.40; p < 0.001). However, the agreement was very significant for tumor type and grade, in which a higher concordance occurred at a higher grade. The percentage of patients with lymph nodes affected was 19.2%;. Although most patients presenting with disease remission or cure were in the early stages (90.5%;), there were no significant differences between those patients who had a misdiagnosis (11/16; 68.8%;) and those who had a correct diagnosis (25/33; 75.8%;) based on preoperative endometrial sampling (p = 0.605). Conclusion Our findings corroborate the literature and confirm the under staging of preoperative endometrial samples based on histopathological assessment, especially for lower grade endometrial tumors. We suggest that the preoperative diagnosis should be complemented with other methods to better plan the surgical management strategy.


Resumo Objetivo Avaliar a concordância entre os diagnósticos histopatológicos de amostras endometriais pré-operatórias e cirúrgicas de pacientes com adenocarcinomas endometriais e avaliar o impacto da concordância entre os diagnósticos no planejamento cirúrgico e sobrevida das pacientes. Métodos Dados de 62 pacientes com câncer de endométrio operadas entre 2002 a 2011 em um hospital universitário foram avaliadas retrospectivamente. As pacientes foram submetidas à histerectomia e tratamento adjuvante, se necessário, e acompanhadas clinicamente de acordo com o protocolo institucional. Foram avaliados os resultados das análises histopatológicas das amostras endometriais pré-operatórias e cirúrgicas. As lesões foram classificadas como tumor endometrioide (tipo 1) graus 1, 2 ou 3 ou carcinoma não endometrioide (tipo 2). Resultados De modo geral, houve uma concordância baixa entre os diagnósticos histopatológicos das amostras endometriais pré-operatórias e cirúrgicas (Kappa: 0,40; p < 0,001). Entretanto, uma alta concordância entre os diagnósticos foi observada nos tumores de graus mais elevados. Comprometimento de linfonodos ocorreu em 19,2%; das pacientes e a maioria das que apresentaram remissão ou cura foram diagnosticadas nos estágios iniciais da doença (90,5%;). Não houve diferença significativa na taxa de remissão ou cura entre as pacientes que tiveram concordância (25/33; 75,8%;) ou divergência (11/16; 68,8%;) entre os resultados histopatológicos pré-operatórios e cirúrgicos (p = 0,605). Conclusão Nossos achados corroboram a literatura e confirmam o sub-estadiamento de amostras endometriais pré-operatórias com base na avaliação histopatológica, especialmente para tumores endometriais de baixo grau. Outros métodos complementares são necessários para um diagnóstico pré-operatório mais preciso a fim de melhorar o planejamento cirúrgico.

Abstract Objective: Blood pressure (BP) references for Brazilian adolescents are lacking in the literature. This study aims to investigate the normal range of office BP in a healthy, non-overweight Brazilian population of adolescents. Method: The Brazilian Study of Cardiovascular Risks in Adolescents (Portuguese acronym "ERICA") is a national school-based study that included adolescents (aged 12 through 17 years), enrolled in public and private schools, in cities with over 100,000 inhabitants, from all five Brazilian macro-regions. Adolescents' height and body mass index (BMI) were classified in percentiles according to age and gender, and reference curves from the World Health Organization were adopted. Three consecutive office BP measurements were taken with a validated oscillometric device using the appropriate cuff size. The mean values of the last two readings were used for analysis. Polynomial regression models relating BP, age, and height were applied. Results: Among 73,999 adolescents, non-overweight individuals represented 74.5% (95% CI: 73.3-75.6) of the total, with similar distribution across ages. The majority of the non-overweight sample was from public schools 84.2% (95% CI: 79.9-87.7) and sedentary 54.8% (95% CI: 53.7-55.8). Adolescents reporting their skin color as brown (48.8% [95% CI: 47.4-50.1]) or white (37.8% [95% CI: 36.1-39.5]) were most frequently represented. BP increased by both age and height percentile. Systolic BP growth patterns were more marked in males when compared to females, along all height percentiles. The same pattern was not observed for diastolic BP. Conclusions: Blood pressure references by sex, age, and height percentiles for Brazilian adolescents are provided.


Resumo Objetivo Referências de pressão arterial (PA) para adolescentes brasileiros estão ausentes na literatura. Este estudo tem como objetivo investigar a variação normal da pressão arterial no consultório em uma população brasileira saudável de adolescentes sem sobrepeso. Método O Estudo dos Riscos Cardiovasculares em Adolescentes (ERICA) é um estudo brasileiro, de âmbito nacional e de base escolar, que incluiu adolescentes (12 a 17 anos) matriculados em escolas públicas e privadas, em cidades com mais de 100.000 habitantes, de todas as cinco macrorregiões brasileiras. A altura e o índice de massa corporal (IMC) dos adolescentes foram classificados em percentis de acordo com a idade e o sexo, sendo adotadas as curvas de referência da Organização Mundial de Saúde. Foram realizadas três medidas consecutivas de PA no consultório com um dispositivo oscilométrico validado, utilizando o manguito de tamanho apropriado. Os valores médios das duas últimas leituras foram utilizados nas análises. Modelos de regressão polinomial relacionando PA, idade e estatura foram aplicados. Resultados Entre os 73.999 adolescentes, os indivíduos sem sobrepeso representaram 74,5% (IC95%: 73,3-75,6) do total, com distribuição similar entre as idades. A maior parte da amostra sem sobrepeso originava-se das escolas públicas, com 84,2% (IC95%: 79,9-87,7), e os sedentários 54,8% (IC95%: 53,7-55,8). Os adolescentes que relataram sua cor de pele como parda (48,8% [IC95%: 47,4-50,1]) e branca (37,8%: [IC 95% 36,1-39,5]) foram os mais representados. A PA aumentou tanto com a idade, quanto com o percentil de altura. Os padrões de aumento sistólico da PA foram mais acentuados no sexo masculino quando comparados ao sexo feminino, em todos os percentis de altura. O mesmo padrão não foi observado para a PA diastólica. Conclusões São fornecidas referências de pressão arterial por sexo, idade e percentil de altura para adolescentes brasileiros.

Chronic renal disease is a common health problem in the world. In 2016, females are mostly affected by CKD than the males. There are many diagnostic methods in modern aspect of medicine like serum Creatinine, urea and etc., The Siddha system of medicine is a treasure dedicates to the world by Siddhars. In Siddha system of medicine, different diagnostic methods present for all diseases. As per Theraiyar, the eight methods of diagnosis ‘Envagaithervu’ are Naadi (pulse), Naa (Tongue), Niram (Color), Mozhi (Voice), Vizhi (Eyes), Malam (Feces), Neer (urine) and Sparisam (Touch). Among the eight diagnostic tools, Neikkuri is one of the important methods for urine analysis. This study was aimed to Observed the Neikkuri (oil on urine sign) in chronic kidney disease patients.40 patients were selected from NIS OPD by non randomised method. The study details were collected in the data collection form with the consent of the patients. Urine samples were collected in glass bowel. A small drop of black sesame oil was instilled on the center of the upper surface of the urine perpendicularly. The spreading nature of the oil has to be observed. Result of the study shows 30% of Neikkuri is Kabapitham nature. This study is helps to evaluate the siddha diagnostic method for Chronic Kidney Disease.

Thus,appropriate choice of diagnostic methods and correct interpretation of the test results can help clinicians to make accurate pathogen diagnosis.

ABSTRACT

Objetivo: evaluar el desempeño clínico de un método de reacción en cadena de la polimerasa con modificaciones para la detección de la replicación del virus de la hepatitis B en pacientes infectados. Métodos: se estudiaron 266 muestras de suero de pacientes provenientes de los servicios de Gastroenterología, Trasplante, Hemodiálisis y Hematología del Hospital Clínicoquirúrgico Hermanos Ameijeiras, con el antígeno de superficie (HBsAg) positivo y altos valores de enzimas hepáticas (aspartatoaminotransferasa, aspartatoaminotransglutaminasa y ganmaglutamiltrasferasa), así como 5 muestras de individuos clínicamente sanos. El ADN se extrajo mediante el método del fenol-cloroformo. Se amplificó un fragmento de la región Pre S del virus de la hepatitis B y un fragmento del gen de la ß-globina como control interno de la reacción. Resultados: El fragmento del gen de la ß-globina se verificó en las 266 muestras estudiadas. La región Pre S del virus de la hepatitis B en cambio, fue detectada en 103 de ellas, siendo informadas como positivas para la replicación viral. En la muestra se obtuvo una prevalencia 98,15 por ciento para la replicación del virus de la hepatitis B. La especificidad clínica del ensayo fue del 100 por ciento, la sensibilidad clínica del 38 por ciento, el valor predictivo positivo del 100 por ciento y el valor predictivo negativo del 64 por ciento. El análisis de concordancia no mostró acuerdo (κ= 0,022 para una p= 0,07) entre las enzimas hepáticas y la reacción en cadena de la polimerasa. Los ensayos de repetitividad y de reproducibilidad mostraron que los resultados son reproducibles. El mayor porcentaje de positividad se encontró en los pacientes remitidos por el Servicio de Hematología (66,7 por ciento). Conclusiones: el método de reacción en cadena de la polimerasa evaluado constituye una herramienta certera para el monitoreo de la replicación del virus de la hepatitis B. Contar con su implementación y aplicación en la institución que se efectuó el estudio reviste gran importancia para el sistema de salud del país(AU)


Objective: evaluate the clinical performance of a modified polymerase chain reaction method to detect replication of the hepatitis B virus in infected patients. Methods: a study was conducted of 266 serum samples from patients cared for at gastroenterology, transplantation, hemodialysis and hematology services of Hermanos Ameijeiras Clinical Surgical Hospital with positive surface antigen HBsAg and high liver enzyme values (aspartate aminotransferase, aspartate aminotransglutaminase and gamma-glutamyltransferase), and 5 samples from clinically healthy individuals. The DNA was extracted by the phenol-chloroform method. Amplification was performed of a fragment of the Pre S region of the hepatitis B virus and a fragment of the ß-globin gene as internal control of the reaction. Results: the ß-globin gene fragment was found in the 266 samples studied. The Pre S region of the hepatitis B virus, however, was detected in 103 of them. These were reported as positive for viral replication. The sample exhibited a prevalence of 98.15 percent for replication of the hepatitis B virus. Clinical specificity of the assay was 100 percent, clinical sensitivity was 38 percent, positive predictive value was 100 percent and negative predictive value was 64 percent. Concordance analysis did not reveal any agreement (κ= 0.022 for p= 0.07) between the liver enzymes and the polymerase chain reaction. Repeatability and reproducibility assays showed that the results are reproducible. The highest positivity percentage was found in patients referred from the Hematology Service (66.7 percent). Conclusions: the polymerase chain reaction method evaluated is an accurate tool to monitor replication of the hepatitis B virus. The possibility of its implementation and application at the institution where the study was conducted is very important for the Cuban health system(AU)

BACKGROUND/AIMS: The incidence and severity of Clostridium difficile infection (CDI) have increased worldwide, resulting in a need for rapid and accurate diagnostic methods. METHODS: A retrospective study was conducted to compare CDI diagnosis methods between January 2014 and December 2014. The stool samples, which were obtained in presumptive CDI patients, were compared for their diagnostic accuracy and rapidity, including real-time polymerase chain reaction (PCR) of toxin genes, C. difficile toxin assay, and culture for C. difficile. RESULTS: A total of 207 cases from 116 patients were enrolled in this study and 117 cases (56.5%) were diagnosed as having CDI. Among the 117 cases, the sensitivities of real-time PCR, C. difficile toxin assay, and culture for C. difficile were 87.2% (102 cases; 95% CI, 80.7%–92.8%), 48.7% (57 cases; 95% CI, 41.0%–59.8%), and 65.0% (76 cases; 95% CI, 60.2%–78.5%), respectively (P < 0.005). Notably, 34 cases (29.0%) were diagnosed with CDI by real-time PCR only. The time required to obtain results was 2.27 hours (136.62±82.51 minutes) for real-time PCR, 83.67 hours (5,020.66±3,816.38 minutes) for toxin assay, and 105.79 hours (6,347.68±3,331.46 minutes) for culture (P < 0.005), respectively. CONCLUSIONS: We confirmed that real-time PCR of toxin genes is the most effective diagnostic method for accurate and early diagnosis of CDI. It also helps to diagnose hypervirulent CDI, such as ribotype 027 infection.

Schistosomiasis diagnosis plays an important role in the schistosomiasis control. The early detection of schistosomi-asis can help to find the infectious source and prevent advanced schistosomiasis effectively. Up to now,serodiagnosis and parasi-tological diagnosis are used commonly to detect the infection of Schistosoma. As the schistosomiasis control program continues in China,the infection rate and infection intensity of Schistosoma japonicum are decreased significantly,which makes the serodiag-nosis and parasitological diagnosis limited for lacking of sensitivity and timeliness. The molecular diagnosis has been developed greatly because of its timeliness,high specificity and sensitivity,which promotes the development and improvement of schistoso-miasis diagnosis. In the endemic areas where schistosomiasis is limited and the infection rate is low ,the molecular diagnosis pro-vides a potential platform for the early detection and micro detection efficiently. Here,we provide a review that mainly emphasiz-es the progress of molecular detection of schistosomiasis.

Background: “The Oral Instruction Tantra” of “Four Medical Tantras” divides diseases in children into 4 parts: causes, condition, categories and symptoms. The diseases are proven with inquiries and examination by touching and looking. The most interesting type of examination is considered to be touching, which the main part is pulse-feeling. However, pulse feeling of children in traditional medicine is different from that of adults.Methods: Document analysis, logical argumentation, analytic-synthetic distinction, grouping and content analysis were used on studying traditional medical books, research literature and other related materials. Results: The general diagnostic method of traditional medicine consists of 3 parts: inquiry, pulse diagnosis and analyzing patient’s urine. The diagnostic method in pediatrics in traditional medicine is based on the general method but different in some ways in consistence with the classification of diseases in children (Four Medical Tantras). Conclusion:The conclusion from the research results are as follows: 1. Inquire the symptoms of children from parents and examine the children’s faces 2. Research the method of examining ear veins, identify its scientific nature and open up possibility to use it in training 3. ABSTRACT

Background: Morbidity and mortality resulting from malaria remains a serious obstacle for social and economic development. Accurate diagnosis and prompt treatment are therefore essential components of case management strategy. The aim of this study therefore was to examine the diagnostic procedure of uncomplicated malaria, and patients’ understanding and satisfaction of treatment in Community Health Care Facilities, three years after the deployment of Malaria Rapid Diagnostic Tests in Ghana. Methodology: A prospective and data collation was done randomly, by means of cluster and stratified multistage surveyat three government hospitals and three private pharmacies in Kumasi, Ghana, between July and September, 2013. Patients treated for uncomplicated malaria, while leaving the health facility, upon consent, were selected and requested to answer questionnaires which served as a source of data to address the objective of the study. Bivariate statistics from the SPSS v 19 was employed to predict the relationships between health institutions and mode of diagnosis, patients’ understanding and satisfaction of services. Results: Fifty-three (53) out of 65 patients responded. The study indicated presumptive diagnosis [44 (83.0%)] to be predominantly used over test-based diagnosis [9 (17.0%)]. The mean age of patients was 34.44±14.8 years (Range 17-66). Out of 52 patients who provided information on educational level, those with tertiary education were 24 (46.2%), secondary were 9 (17.3%), primary were 14 (26.9%) and no formal education were 14 (26.9%). Male patients were 25 (47.2%) and female 28 (52.8%). All 53 patients were given Artemisinin-based Combination Therapy at the various health facilities. Of 35 patients at hospitals/clinics, 15 (42.9%) rated “very good value” to explain their understanding and satisfaction of services provided, and of 18 patients from private pharmacies, 10 (55.6%) rated as “very good value”. Patients with tertiary education [14/25 (56.0%)] showed better understanding and satisfaction of services than those with no formal education [1/25 (4.0%)]. Not a single use of Malaria Rapid Diagnostic Tests for diagnosis was recorded. Conclusion: Diagnosis of malaria at the periphery of health systems is still mainly presumptive three years after deployment of the Malaria Rapid Diagnostic Test. Patients’ good rating on the diagnosis of uncomplicated malaria at private pharmacies, should be an advantage to introducing the Malaria Rapid Diagnostic Tests by healthcare practitioners.

[Result]Before modern times, the creative technology of TCM diag-nosis and treatment in Zhejiang was characterized with early start, long maintenance and wide range, moreover, in the clinical sections of internal medicine, gynecology, paediatrics, orthopaedics and rhinology, the new theory and new technology continual y came into being on treating rules, methods and diag-nosis. Therefore, it promoted the diagnosis and treatment levels in the whole. [Conclusion] Before the modern times, the creation of Zhejiang diagnosis and treatment technology originated from many doctors ’medical practices; the various diseases sourced from Zhejiang unique geographical and climatic environment offered good stage for generations of doctors to distinguish themselves; and the developed society and material civilization in Zhejiang area provided strong guarantee for Zhejiang doctors to find and invent new diagnosis and treatment technology.

ABSTRACT

Ascidian soft tunic syndrome (AsSTS) caused by Azumiobodo hoyamushi (A. hoyamushi) is a serious aquaculture problem that results in mass mortality of ascidians. Accordingly, the early and accurate detection of A. hoyamushi would contribute substantially to disease management and prevention of transmission. Recently, the loop-mediated isothermal amplification (LAMP) method was adopted for clinical diagnosis of a range of infectious diseases. Here, the authors describe a rapid and efficient LAMP-based method targeting the 18S rDNA gene for detection of A. hoyamushi using ascidian DNA for the diagnosis of AsSTS. A. hoyamushi LAMP assay amplified the DNA of 0.01 parasites per reaction and detected A. hoyamushi in 10 ng of ascidian DNA. To validate A. hoyamushi 18S rDNA LAMP assays, AsSTS-suspected and non-diseased ascidians were examined by microscopy, PCR, and by using the LAMP assay. When PCR was used as a gold standard, the LAMP assay showed good agreement in terms of sensitivity, positive predictive value (PPV), and negative predictive value (NPV). In the present study, a LAMP assay based on directly heat-treated samples was found to be as efficient as DNA extraction using a commercial kit for detecting A. hoyamushi. Taken together, this study shows the devised A. hoyamushi LAMP assay could be used to diagnose AsSTS in a straightforward, sensitive, and specific manner, that it could be used for forecasting, surveillance, and quarantine of AsSTS.

ABSTRACT

<p><b>OBJECTIVE</b>To study the technique of Western blot for the diagnosis of Lyme disease caused by Borrelia afzelii in China and to establish the standard criteria by operational procedure.</p><p><b>METHODS</b>FP1, which is the representative strain of B. afzelii in China, was analyzed by SDS-PAGE, electro transfer and immunoblotting assays. The molecular weights of the protein bands of FP1 were analyzed by Gel-Pro analysis software. In a study using 451 serum samples (159 patients with Lyme disease and 292 controls), all observed bands were recorded. The accuracy of the WB as a diagnostic test was established by using the ROC curve and Youden index.</p><p><b>RESULTS</b>Criteria for a positive diagnosis of Lyme disease were established as at least one band of P83/100, P58, P39, OspB, OspA, P30, P28, OspC, P17, and P14 in the IgG test and at least one band of P83/100, P58, P39, OspA, P30, P28, OspC, P17, and P41 in the IgM test. For IgG criteria, the sensitivity, specificity and Youden index were 69.8%, 98.3%, and 0.681, respectively; for IgM criteria, the sensitivity, specificity and Youden index were 47%, 94.2%, and 0.412, respectively.</p><p><b>CONCLUSION</b>Establishment of WB criteria for B. afzelii is important in validating the diagnostic assays for Lyme disease in China.</p>

ABSTRACT

We elaborated an alternative culture method, which we denominated PKO (initials in tribute of respect to Petroff, Kudoh and Ogawa), for isolating Mycobacterium tuberculosis from sputum for diagnosis of pulmonary tuberculosis (TB), and to compare its performance with the Swab and Petroff methods. For the technique validation, sputum samples from patients suspected of pulmonary TB cases were examined by acid-fast microscopy (direct and concentrated smear), PKO, Swab and Petroff methods. We found that Petroff and PKO methods have parity in the effectiveness of M. tuberculosis isolation. However, by the PKO method, 65 percent of isolated strains were detected in a period of £15 days, while by the Petroff method the best detection was in an interval of 16-29 days (71 percent). In positive smear samples, the average time of PKO isolation is only superior to the one related for Bactec 460TB. In conclusion, the exclusion of the neutralization stage of pH in the PKO reduces the manipulation of the samples, diminishes the execution time of the culture according to the Petroff method and facilitates the qualification of professionals involved in the laboratorial diagnosis of Tuberculosis.


Foi elaborado um método de cultivo alternativo, denominado por nós PKO (iniciais referentes à Petroff, Kudoh e Ogawa), para o isolamento do Mycobacterium tuberculosis em amostras de escarro para o diagnóstico da tuberculose pulmonar (TB). Para validação da técnica, amostras de escarro de pacientes suspeitos de TB foram submetidas aos métodos de baciloscopia (direta e pós-concentração), PKO, Swab e Petroff. A análise comparativa entre o método de Petroff e o PKO mostrou paridade de resultados em relação ao isolamento e número de colônias de M. tuberculosis. Porém, pelo método PKO, 65 por cento das cepas isoladas foi detectada em um período £15 dias, enquanto que pelo método de Petroff a melhor detecção ocorreu em um intervalo de 16-29 dias (71 por cento). O tempo médio de isolamento pelo PKO é somente superior ao sistema comercial Bactec 460TB em amostras positivas na baciloscopia. A exclusão da etapa de neutralização de pH no método PKO reduz a manipulação das amostras, diminui o tempo de execução do cultivo em relação ao de Petroff e facilita o treinamento de profissionais que realizam o diagnóstico laboratorial da TB.

ABSTRACT

Objective To explore the image characteristics and clinical application of the optical coherence tomography (OCT) in patients with epiretinal membranes of the macular(ERMM). Methods 38 patients, who were diagnosed as or suspected as ERMM, were examined with OCT before and after operation from November, 2002 to October, 2003 in our hospital. Results Macular epiretinal membranes were visible on OCT as high reflective tissues, which were thin or thick, and contiguous to or anterior to the retinal surface. In most fovea, the depth decreased and the thickness increased. ERMM disappeared after operation. Conclusion OCT can display the macular epiretinal membrane and the pathological changes of macular tissues before and after operation. OCT can provide accurate information on the clinical diagnosis and operative efficacy of ERMM.

ABSTRACT

This study was conducted to evaluate the health hazards and to develop diagnostic methods of glass fiber workers. We examined 40 male glass fiber workers (exposure group) and 57 male non-glass fiber workers (reference group) with a questionnaire, physical examination, chest x-ray and pathological examination in Mar, 1997. Also we examined 65 male glass fiber workers (exposure group) and 42 male non-glass fiber workers (reference group) with the same methods also we did some energy-dispersive x-ray analyses with a scanning electron microscopic examination in Sep. 1997. Most of the clinical symptoms were significantly more frequent among the exposure group than the reference group. Coughing (32.5%), itching of the nose (30.0%), irritation of eyes (27.5%), irritation of the nose (25.0%) and sputum (22.5%) were the major symptoms among the exposure group in Mar. 1997. Only itching of the nose was significantly more frequent in Sep. 1997. No cases of pneumoconiosis were observed among the groups and there were no differences in chest abnormalities between the exposure group with the reference group on both examinations. The prevalence of dermatosis among the exposure group was 20.0% (8 cases) and the cumulative prevalence was 72.5% (29 cases) in Mar. 1997. The prevalence of dermatosis among the exposure group was 23.1% (15 cases) and the cumulative prevalence was 58.5% (38 cases) in Sep. 1997. Onset of dermatosis among the exposure group was most frequent within one month after handling. The frequent sites of skin lesions were the hands and arms on both examinations. Glass fiber induced skin lesions can be diagnosed by the scotch tape method or KOH mount and then can be examined under the light, polarizing and phase-contrast microcopies. Glass fibers can be identified by some energy-dispersive x-ray analyses with a scanning electron microscopic examination.

ABSTRACT

BACKGROUND: Identification of the histologic cell type of lung cancer is important because it is related to the treatment modality and prognosis. Currently, many diagnostic methods are used to determine the cell type. We have studied to evaluate the histologic accuracy of each diagnostic methods in lung cancer. METHOD: 168 cases were analysed retrospectively, who underwent curative thoracotomy for lung cancer in Seoul National University Hospital from January, 1994 to Feburary, 1997. Sputum cytology, percutaneous needle aspiration(PCNA), percutaneous needle biopsy(PCNB), bronchial washing cytology and flexible bronchoscopic biopsy were evaluated respectively. The kappa coefficient was applied to evaluate the degree of concordance between the histologic diagnosis obtained by each methods and the one derived from thoracotomy. RESULTS: 1. The kappa value in sputum cytology was 0.86 In the 34 cases with squamous cell carcinoma(SQ), 32 cases were finally diagnosed as having such by thouacotomy. 7 of the 8 cases with adenocarcinoma(AD) turned out to have the same. 2. The kappa value in PCNA was 0.51. In the 31 cases with SQ, only 14 cases were finally diagnosed as having such. All of the 3 cases with small cell carcinoma(SC) turned out to have the same. 3. The kappa value in PCNB was 0.77. The diagnosis was correct in 13 of the 16 patients with SQ and in 30 of the 32 cases with AD. 4. The kappa value in bronchial washing cytology was 1.0. In all of the 29 cases with SQ and all of the 7 cases with AD, the diagnosis was correct. 5. The kappa value in flexible bronchoscopic biopsy was 0.77. The diagnosis was correct in 51 of the 52 cases with SQ and in 1 of the 2 cases with SC. CONCLUSION: The concordance rate with the final histologic diagnosis in sputum cytology, PCNB, bronchial washing cytology and flexible bronchoscopic biopsy were excellent (kappa >or=0.75), while that in PCNA was fair (kappa=0.53). Because PCNA showed lower concordance rate than other diagnostic methods, PCNA is recommended to perform with PCNB.

ABSTRACT

There has been a marked increase in the utility of aspiration cytology for pathologic diagnosis. It may be applied to any kinds of organs and substitutes surgical biopsy. Because of the high risk of complication and difficulties in localization, aspiration cytology in the central nervous system(CNS) has been used with less frequency compared to other sites. However, with the advent of sophisticated imaging instruments, aspiration cytology of lesions in the CNS is being used increasingly. Cytologic features of the CNS neoplasms were quite similar to those of histology except one spindle cell tumor. Reviewing various CNS neoplasms, it appears that cytology may be a useful diagnostic method.
